CCB Navigator

The Community Care of Brooklyn (CCB) Navigator connects Brooklynites with over 300 community-based social, behavioral, and medical care services and programs, offering individuals the resources they need to stay safe, access quality services and avoid health crises.

This “one-stop-shop” care management service is free and available to all Brooklyn residents: from the emergency room, to inpatient and outpatient settings, to the community at-large, regardless of insurance or ability to pay.

What Happens When You Call the CCB Navigator?

Referrals are made only to free resources that individuals are interested in receiving, and there is no obligation to accept services or proceed with the next step in the process. The CCB Navigator cannot assist with clinical oversight or concerns such as billing, insurance, prior authorizations, and medication reconciliation.
